The practical question with a colostomy is whether output is formed enough for a closed pouch. If you are changing once or twice a day and output is solid, a closed pouch is simpler: nothing to empty, no clip, and the pouch is just replaced. Hollister closed pouches come in Midi and Maxi in 45, 55 and 70 mm; the HDL-Hendry closed pouch is 60 mm.

If output is looser, or if you are emptying more than twice a day, a drainable pouch is less work and gentler on the flange. It is common to use closed pouches most days and keep drainables for periods when output changes — after a course of antibiotics, or a change in diet.

Most colostomies sit on reasonably flat skin and do well on a flat skin barrier. A barrier ring is worth adding if the skin near the stoma has a scar or a dip, and convexity is reserved for a stoma that is flush with the skin or below it.

Wear time depends on output, skin and activity rather than on a fixed rule. Change the barrier when you feel itching or burning underneath rather than waiting for a leak — discomfort under a wafer is nearly always output reaching skin.

Odour worries most people more than anything else early on. Pouches carry filters that release gas gradually, and a lubricating deodorant inside the pouch helps at the moment odour is most noticeable — when you open or change it. Persistent odour between changes usually means the seal is leaking air somewhere, which is a fitting question rather than a deodorant one.

In the first months after surgery the stoma shrinks. Re-measure it every few weeks and adjust the opening you cut, rather than reusing the same template — an opening that was right in week two is often too large by week ten.
